It takes some effort and a little knowledge to fix the problem. There are some simple exercises and tools that can help also. The two reference links are from the same site, but you should be able to get everything you need to make a difference.
References :
http://www.necksolutions.com/back-posture.html

I'm the same way. I always sit slouched. Your back isn't supposed to be 'straight'. Your spine makes an 'S' shape. When you're in class just try to remember to sit up. Or if you're sitting around the house. Just make it a habit, like you when you got used to slouching. xD
References :
I had the same problem when I was young. I was very tall as a kid. I made a mental note to stand tall and reminded myself all the time. If you keep doing this you will be hunched-back like my brother, (he's 6'5) Please walk tall and sit up straight. :)
References :
Experienced it

first of all, to help fix ur posture u can hang upside down (if possible) - this will help ur back straighten itself... as well as decompressing some of the cartilage between ur bones - this will also help u gain height.. added bonus if u ask me

then to maintain ur posture u could get a firm mattress on ur bed (tho recent scientist claims are these are useless - i sleep better on a firm mattress) - do stretching exercises... sit up straight... 2 feet on the floor at all times where possible (takes weight off ur lower back)... place ur butt into the back of ur seat and try to keep ur back straight - this will also help prevent slipped discs etc in later life
References :
im a trained physician

there are braces online that you can buy that will pull your shoulders back and keep you sitting up straight. I have found though the best brace are the ones you get at a hospital for a broken collar bone called a figure 8 brace. Also working out your abs is good for your posture because your abs support your back
